Tuberculosis is a major public health problem in many countries. The World Health Organization (WHO) estimates that nearly two billion people, or 40% of the world""s population, are infected with the tuberculosis bacillus, Mycobacterium tuberculosis. The WHO also reports that about 1.8 million people died from tuberculosis in 1998, of which about 0.4 million were children under the age of 14. In spite of the recent increase in the number of cases of tuberculosis, the use of a conventional smear technique in a clinical laboratory, such as the Ziehl-Neelsen method, for detecting mycobacteria remains the gold standard. Typically, a sputum smear is stained by Ziehl-Neelsen and examined by light microscopy for the presence of acid-fast bacilli (AFB), the most significant cells among those harvested from the sputum of a patient. In one commonly used method, the specimen is smeared onto a slide and heated on an electric hotplate at about 100xc2x0 C. for about 15 to 20 minutes. While the Ziehl-Neelsen method is readily accepted by the medical community, the technique is far from ideal. In addition, the sensitivity of the method is such that it is possible to have false negatives. These false negatives often occur when smears are stained too early after collection, when the bacilli are destroyed. In addition, some TB cases are X-ray positive (clinically evident) but mycobacteria-negative in sputum smears, or vice versa. Such cases present a significant problem, particularly in countries where drug-resistance is high. Moreover, since positive findings on a smear are normally confirmed by culturing the organism on an agar-containing media, there is also a large window of time in which the positive smear may actually be false.
